Step 12 - Submit the Certification

Submit the completed and signed Certification to your Vice President, Dean, Director, Department Head or Head of Center.


Vice Presidents, Deans, Directors, Department Heads or Heads of Centers collecting Certification, Critical Device Registration  and Exception forms should complete a Manager Confirmation form and return it, along with all forms, to:

NOTE:  Please do not fax your certification forms and manager confirmation form to our office.  Please send the forms via campus mail or courier, or drop them off at the Computer Center.  Thank you.
